The links below allow you to download PDF files of the Official Miss California Organization contestant and local pageant forms.

You will need Adobe Acrobat Reader (Version 7 or higher) to open these files on your computer and save the completed form. To download and install the most current version of the free Adobe Acrobat Reader, click here.

These forms are designed for use on a computer, but can also be filled in on a typewriter.

If you are using any version of Adobe Acrobat Reader that is version 6 or lower, you will NOT be able to save the completed PDF form with the entered data; you must print it to retain a copy of the data.

Suggested Ways to Use these Forms:

To complete these forms using a computer...

1) - Download PDF files to your computer and save them on your hard        drive or a Zip disk or floppy disk.
       To download files directly to your hard drive - without displaying        them on the screen - right click, and select the
        'Save Target As...' option.
2) - Open downloaded file in Adobe Acrobat Reader (Version 7 or        higher).
3) - Type in information in blanks indicated on the form.
4) - Use the 'Save as ...' option on the FILE menu to save the       completed form to your hard drive with a new file name.
5) - Print out the completed form on your printer
6) - Proofread the completed form. Make changes on screen as needed,       and reprint.
7) - Don't forget to save again, when all of the information is correct.       Then exit the program.

NOTE: After completing the last field on any of the forms, click away anywhere else on your screen. Otherwise, what is typed in the last field doesn't print out.

To complete these forms using a typewriter

1) - Download PDF files to your computer and save them on your hard       drive or a Zip disk or floppy disk.
      To download files directly to your hard drive - without displaying       them on the screen - right click, and select the
       'Save Target As...' option.
2) - Open downloaded file in Adobe Acrobat Reader.
3) - Print out the blank form. Exit the program.
4) - Complete the form using a typewriter.
